If you have anyone of these insurances and are wanting to confirm if we take it please call the office or email us with the following information along with a picture of the front & back of the insurance card:

  • The patient full legal name
  • Date of birth
  • Name of insurance company
  • Complete member ID
  • Group number
  • Phone number for eligibility/ benefits.

Will Synergique Healing verify my insurance coverage before my first appointment?

Our staff will verify your benefits prior to scheduling your first appointment. During this process your insurance carrier will provide us with an estimated cost of your visit (this is also known as a general quote of benefits). Please be aware that this is only a general quote of benefits and is subject to change until your insurance carrier processes your claim. This also applies to established patients who have changed insurance carriers or plans.

Are there fees for other services provided by Synergique Healing?

Additional fees are assessed for specific services (e.g., completion of forms, letters, legal testimony, etc.), late cancellations, no-shows, or abuse of the emergency line. If you have a question regarding a specific fee please call the office.

When will I be expected to make my payment?

Payment of copayments, coinsurance, deductibles, or self-pay rates are due in full at the time of service. Waiver of these dues may constitute fraud under state and federal law. We will have no choice but to reschedule your appointment if you are unable to pay for your visit at the time of service (except in the rare case of psychiatric emergencies).
